Hometown medical services team leads our care teams and collaborates with skilled nursing communities across their continuum of care, serving and coordinating all facets of medical care. Hometown oversees and advises on medical care processes and procedures and also directs quality improvement programs and overall management of the on-site provider care teams. Hometown provides clinical care training and mentoring, and advises community management teams on best practices.

Our partner facilities collaborate with Hometown to provide resident-patients personalized physician-led care coordination, throughout all levels of their in-home care needs, from independent living to assisted living & memory care to skilled nursing.
